VISION 

The Municipal Civil Registrar’s Office is a recognized Municipal Service provider of accurate Civil Registration products and services.

MISSION

                As a primary Registry Arm of the Local Government, the Municipal Civil Registrar’s Office aims to Provide timely accurate and reliable information as basis for plans, development, to establish and preserve the basis for legal status of Filipinos through efficient use of its human and non-human Resource in a systematic conduct of an effective System of Civil Registration.

REGISTRATION OF VITAL EVENTS CONCERNING THE STATUS OF A PERSON

REBUPLIC ACT NO. 3753 mandates the establishment of a civil register in the Philippines where acts, events, legal instruments and court decrees concerning the civil status of persons shall be recorded.

The birth of a child, being a vital event for a person, must be registered within 30 days from the time of birth at the Office of the Civil Registrar of the city/municipality where the birth occurred.

For ordinary marriages, the time of submission of the Certificate of Marriage is 15 days following the solemnizing of marriage. For marriages exempt from the license requirement, the prescribed period is 30 days.

DUTIES AND FUNCTIONS OF THE OFFICE OF THE MUNICIPAL CIVIL REGISTRAR

  1. Accept all registrable documents and judicial decrees affecting the civil status of the persons;
  2. File, keep and preserve in a secure place the books required by law;
  3. Transcribe and enter immediately upon receipt all registrable documents and judicial decrees affecting the civil status of persons in the appropriate civil registry books;
  4. Transmit to the Office of the Civil Registrar-General, within the prescribed period, duplicate copies of registered documents required by law;
  5. Issue certified transcripts or copies of any certificate or registered documents upon payment of the prescribed fees to the treasurer;
  6. Receive applications for the issuance of a marriage license and after determining that the requirements and supporting certificates and publication thereof for the prescribed period have been compiled with, issue the license upon payment of the authorized fee to the treasurer;
  7. Coordinate with the National Statistics Office in conducting educational campaigns for vital registration and assist in the preparation of demographic and other statistics for the local government unit.


HOW TO AVAIL SERVICES?

  1. REGISTRATION ON CERTIFICATE OF LIVE BIRTH  

For On Time Registration: Prepared with requirements (30 days from birth)

  • Community Tax Certificate(CEDULA) of parents
  • Marriage Certificate of Parents

Process:

2 Pay the corresponding fee to the Municipal Treasurer’s Office and present the Official receipt to the Office of Municipal Civil Registrar Receive and sign the prepared certificate of live birth. For Delayed Registration: ( 10 days posting)

  • Certificate of no record/Negative result from the Philippine Statistics Authority(PSA)
  • Affidavit of Disinterested Person duly signed by two witnesses
  • Bring any two(2) of the documents below:

Immunization card/Baptismal Certificate/Voters Certification/School Records of Form 137/Postal ID/SSS Id from RS-1/Service Record and any documents reflecting the personal data of the applicant.

  • Community Tax Certificate(CEDULA) of parents
  • Marriage Certificate of Parents

Process:

4 After 10 days claim the owners copy of Certificate of Live Birth Receive and sign the prepared certificate of live birth.
